I'm just starting to model for the ioquake engine and i'm curious about which tools you guys use for editing MD3 files. Do you do all material stuff inside blender and just export it? What about Misfit Model 3D?

I use Blender 2.49.


Misfit Model 3D's totally fine, except for player models as that requires much higher demand for animation control like IK, etc.

You can even use MilkShape 3D, although they still ask a fee to use it. It has lesser features as Blender, but more than MisfitModel3D.

I would not recommend Milkshape 3D for anything at all.

What are the problems with the blender exporter? Is it better to export to Misfit Model and then export from there?

The Blender 2.49 MD3 exporter found here (http://leileilol.mancubus.net/quackstuff/blender/) is the most mature and featured implementation.  The MD3 exporters for later versions of Blender leave out tags since they were developed for non-idtech3 engines (Gzdoom, Eduke32, etc)

At least it has a better exporter than Blender...

Milkshape 3D is always the recommended software from the inexperienced.  You are not aware of its underlying faults, like trying to construct a single triangle to make a tag, animating and then suddenly your bone scales for no reason, also the strict viewports and the UV editor are a joke.  Also to export a MD3 you'd need to write a .QC file.  Milkshape teaches bad habits and is a heavy skill limiter.

just after reading your post, I, retried the download with Firefox but, like yesterday, your link redirected to this error page (http://www.katsbits.com/htm/hotlinkerror.php).

I retried with Msys, that returned an error message because it haven't wget, then with Cygwin, that downloaded succesfully the archive. Apparently this link don't work from a web navigator.

It's not my link, I just did a quick check. Given the referrer check, maybe you need to click the same download link (MD3 export for 2.63a with "tag_" support) in this forum post (http://www.katsbits.com/smforum/index.php?topic=275.msg1522#msg1522)?
