Error: Bad Item Index 42 on Entity

I was recently given a demo and its trying load map 'ps5ctf'. Once it loads, i get the error stated in the subject. I have also seen this message when I try to access a certain map on someone server. Is this something I need to do on my end or is this a server side setting that needs adjusting? Also, if someone can explain what this means. I assume its a line of code that isnt corresponding well with the game.

I assume your stating that some thing is wrong with the map. Any idea how I can view this demo and get around the error?

0kelvin's link was not helpful (that error message is not there).

I can only guess what the error means, and my guess is that
the demo was made using a certain mod and you need to have
and activate the same mod to play it (a mod which has more
items than normal).

It means that u are/were not using the standard OA081 VM.
