use a custom skin: http://ioquake3.org/models/
you can find a bfg there and also other weapons, its not 100%compatible ..but I would say 99%.
the skin will only work on unpure servers, or on server having it also. Or when you play singleplayer..

Or use Blender, create a model yourself and release it under GNU GPLv2 or later.

however, if you change a weapons model, you will not see it if you are in a pure server
