how do I add those to OpenArena, and will it mess up the game if I do,

You just add the .pk3 files to the BASEOA folder, then start the game. If they don't appear in the model select screen, then close and restart the game.
Using custom models won't mess your online gaming experience, BUT chances are you'll be the only one seeing your customized appearance. Others won't unless they also have the model on their computer.

BTW, Pure servers only let you use the files that they have.

To create maps you need GtkRadiant.
To create models you need Blender3D or another modeler of similar functionality.
To create 2D content such as textures and skins you need the GIMP or similar.

You'll also need to spend some time learning how to use the above, and that's the difficult part.

To bastardize a quote of Paul Jaquays, you just have to put the same amount of effort that you spend in mastering a game into learning to use the tools to create assets.
